The Outreach Primary Care team is an interprofessional collaborative group that includes a physician, nurse practitioners, nurses, outreach workers, and a social worker, that delivers full scope primary health care and social care to vulnerable clients who are experiencing homelessness and insecure housing.
This dedicated team strives to ensure equity and access for clients to healthcaring and connect to services such as housing, health care resources, living and legal support services, and income resources. The team works from a trauma-informed framework and endeavors to be adaptable, creative, and innovative in their approach to break down/reduce barriers to healthcare.

Kitchener Public Library
Provides health coverage and supporting ID to 800+ people/year. Most clients are experiencing homelessness (who use our address for OHIP), but we also support clients at-risk of homelessness who live on a low, fixed income.
ID is necessary to have access to healthcare, and it needed to obtain housing and income. Getting and keeping ID is difficult for unhoused people who lack a secure place to store it.
The ID Clinic helps people obtain an Ontario Health Card and, when necessary, apply for a birth certificate or proof of status in Canada. These documents then support an application for other ID.
We don’t support replacement of passports, Permanent Residence cards, marriage, death or citizenship certificates.
Outreach locations include: HOF, St. John’s Kitchen, King Street Shelter, encampments, Better Tent City, Women’s Crisis Services, Motels, Ray of Hope, Grand River Hospital and St. Mary’s General Hospital.
We see clients by drop-in and appointment at 44 Francis Street South, Kitchener.
