Social Prescribing

 

Social Prescribing is about supporting the health and well-being of people by using community-based activities and support.  Like a medication prescription, your doctor is recommending a treatment – social connection! Social prescribing bridges the gap between the social and medical models of health and wellness.

Social prescribing is beneficial if you feel that you need some support to mind your health and wellbeing or if you feel isolated, stressed, anxious or depressed.  Using resources within the community will help you to benefit from meeting other people or taking part in a new activity!

Food Security

 

At Community Healthcaring Kitchener-Waterloo, we strive to build food security in our community.  Food security exists when all people, at all times, have access to enough safe and nutritious foods that meet dietary and cultural needs to lead an active and healthy life. 

 

With support from the TD Friends of the Environment Foundation Grant, Community Healthcaring KW has 5 raised garden beds on the corner of Francis Street South and Joseph Street, at the front entrance of the Francis St location.  The goal of these beds is to grow produce and educate on urban gardening.

Pay-What-You-Can Community Market!

We are excited to announce that Community Healthcaring KW will host a new Community Market initiative. This sustainable food with dignity program follows the “pay what you can” model, with access being to all populations, community members, and individuals, regardless of food status.  With support from various local agencies and food rescue programs, we can provide all community members with access to nutritious foods.

 

 

Digital Equity

 

Digital equity is an issue that affects health outcomes in numerous ways. The digital divide has widened the gap between different communities, leading to disparities in healthcare access and outcomes. Those who lack access to digital tools and resources are often left behind in terms of health education, prevention, and treatment. This can have a profound impact on the health and well-being of individuals and communities.

 

At Community Healthcaring Kitchener-Waterloo, clients have access to a four-week tablet lending program. Individuals who may not have access to a personal computer or smartphone can borrow a tablet and access telemedicine services, which can greatly improve their ability to receive timely care. The tablets can also be used to access online resources such as virtual support groups and programming, as well as staying connected with loved ones. All clients who access the tablet lending program have the opportunity to receive one on one support with learning how to use the device. Sessions can include using ZOOM, email, as well as searching the web.
