The Diabetes Education Program at Community Healthcaring Kitchener-Waterloo is one of several sites in the Waterloo Region. We receive referrals from the Waterloo Wellington Diabetes Central Intake Program. We are a client centered and education driven program. We encourage clients to lead their own health journey and are committed to providing high quality care to those who are impacted by the social influences on health.
