For people experiencing issues of homelessness or precarious housing, mental health, and substance use, it can be very hard to navigate the healthcare system. The Hart Hub Waterloo Region is designed to provide people with guidance, resources, and support to meet their individual needs, as well as the necessary information to make decisions about their health and housing journey. With this support, individuals can feel more connected and able to make their own choices, getting the right care, when they choose.

 

Through strong partnerships with health and social service agencies across the region, the HART Hub will provide increased, coordinated access, and expanded hours to a full suite of programs and services for anyone experiencing issues of homelessness or precarious housing, mental health, and substance use.

Hart Hub Waterloo Region will not directly replace the Safe Consumption and Treatment Services. The anchor location at 44 Francis Street South does not offer the full suite of Hart Hub Services but will be a location for referral into services.  

 

For change oriented individuals, Hart Hub Waterloo Region will provide caring and coordinated supports to meet immediate needs and help people build stability while facing housing, mental health, and substance use concerns in Waterloo Region.

The following services will be in place:

• Intake
• Care Coordination
• Additional Rapid Access to Addiction Medicine (RAAM) clinic hours and location at 44 Francis St. S.
• Existing Primary Care and Social Support Outreach services at SJK, Ray of Hope, encampments, shelters, Erb Village, Sheltercare and Supportive Housing
• Employment Services (The Working Centre)

Scaled Services to include:

• Outreach drop-in clinic hours at 44 Francis St. S.
• 5 day/week interprofessional team (primary care, social supports, addiction counselling, mental health counselling)/RAAM at Ray of Hope
• Mobile outreach services evening/weekend
• Food, laundry, showers 5 days/week Ray of Hope
• 12 Transitional Beds (abstinence based)
