Community Healthcaring Kitchener-Waterloo conducts an annual Client Experience Survey to check in with clients, to learn what we are doing well and where there are opportunities to improve.  Survey feedback helps us ensure we are meeting client needs and providing the best healthcaring possible.   

Health Centre clients are invited to access this short survey in the language you are most comfortable with, via the links provided, or if you prefer via the QR codes below.  If you would like to complete a survey and are experiencing a barrier to doing so, please speak with a staff member for assistance.   Survey results are anonymous and confidential.
